The first thing you’ll be asked is whether you want to share your contact details or fill the form in anonymously.
If you select anonymously you've be asked:
- Describe what has happened. You will have a list of behaviours to choose from, you can select multiple answers, I’m not sure or ‘Other’ and describe in your own words.
- If the incident happened to you or someone else, you can also choose to prefer not to say.
- A free text box to give more information in your own words.
- How long have you been experiencing this, and where has it been happening?
- Your relationship with the organisation, which part of the organisation you are working in and who you want to report to go to/who you would prefer a response from.
- The person whose behaviour you are reporting, and whether they are in the same department as you.
- Any factors related to your personal characteristics that played a role in your experience?
- Your reasons for reporting anonymously you can choose from a list or other and describe in your own words.
- If you would like to opt into continued communication. You can read more about this on our dedicated support page for two-way anonymous messaging. [Insert link]
- Questions about your demographics, which are optional but encouraged, to help us understand who is reporting.
If you select with contact details you'll be asked:
- If the incident happened to you or someone else, and you can also choose to prefer not to say or other.
- Fill in your name, pronouns and how you would like to be contacted.
- Describe what has happened. You will have a list of behaviours to choose from, you can select multiple answers, I’m not sure or ‘Other’ and describe in your own words.
- A free text box to give more information in your own words.
- How long you’ve been experiencing this, and where it’s been happening.
- Your relationship with the organisation, which part of the organisation you are working in and who you want to report to go to/who you would prefer a response from.
- The person whose behaviour you are reporting, and whether they are in the same department as you.
- Any factors related to your personal characteristics that played a role in your experience?
- If you want to make a formal report, details of what this means can be found on the support pages.
- Questions about your demographics, these are optional but encouraged to help us understand who is reporting.
You will then have the opportunity to review and change any of your answers and submit your form. Once submitted, you can download the report.
